Answer: My pickup never had one either. I aligned it to where I wanted it, drilled pilot holes, then installed it. Works perfectly. It's been 8 years already. -

How many bolts to attach it to the door? Does it have threads in studs to bolt it in?
Answer: It has two bolts that thread into existing anchors on the door frame. -

I never had a passenger side mirror. Any tricks to position it and what kind of screws to use?
Answer: I took off the driver's mirror. Pick a reference point, edge of a small window, etc. I used masking tape. Basically, make a template and transfer it over to the passenger side. Done. -

Does this mirror have a 2-bolt or 3-bolt pattern to hold it to the door? Does it have threads in the studs that fit in the door to bolt it on?
Answer: Mine had 2 screws. They lined up well with the mounts on my 1990 4Runner. The mirrors themselves were slightly smaller than OEM but were easy to get used to and looked fine.
